CPAFH Examination

The Certified Professional in Age-Friendly Health Care (CPAFH) examination is a computer-based test offered both in person at PSI Assessment Centers and online via live remote proctor.

The examination for the Certified Professional in Age-Friendly Health Care (CPAFH) credential is offered throughout the year at PSI Assessment Centers located domestically in the United States and internationally. The examination is also available online via live remote proctor.

The examination is administered by appointment only.

Available dates will be indicated when you schedule your examination. Candidates are scheduled on a first-come, first-served basis.

Candidates will be given 2.5 hours to complete the examination.

  • Age-Friendly Assessment, Principles, and Considerations
  • Age-Friendly Interventions, Education, and Counseling
  • Age-Friendly Framework, Practices, and Design

How to Book a CPAFH Examination

The CPAFH examination is administered by appointment only at a PSI Assessment Center or online via live remote proctor.

  1. Visit the PSI certification website
  2. Use the link in the top right corner to sign in or create an account
  3. Click on View Available Tests
  4. Select Certified Professional in Age-Friendly Health Care (CPAFH)™ Examination
  5. Click the Apply Now button
  6. Complete the fields in the application. If you need to save your progress to return later, there is an option to Save Application at the bottom of the page. When finished, click the Submit Application button.

Once your application has been submitted, it will need to be reviewed. This can take up to two weeks. Upon approval, you will receive an e-mail confirming your eligibility and inviting you to schedule your test.

Use the same e-mail and password that you created when applying to access your account. You will be able to view available tests, select the test for which you are eligible, and schedule.

Scores from examinations taken during the initial beta testing period (October 1–November 30, 2025) will be held until February 2026 to allow for standard-setting procedures and review. Please note that examinations will not be administered during the blackout period (December 1, 2025–January 31, 2026). However, exam scheduling will remain open throughout this blackout period.

Live Remote Proctoring for CPAFH Exam

Live remote online proctoring option for the CPAFH exam allows CPAFH candidates to continue advancing their personal and professional goals without having to travel to a testing center. See the “How to Book an Examination” section for more information.

Please be advised: Using a work computer for the remote proctored exam, even one which passes the System Compatibility Check, is highly discouraged due to administrator firewalls and IT security systems which frequently prevent exam launch. International Candidates

Candidates testing outside of the US may submit their applications online or by mail. International candidates will be notified of the exam scheduling process as follows:

  • Online applicants will be notified by email as soon as they complete the application.
  • A candidate who submits a paper application form will be notified by email within 10 business days of receipt of his or her application form.

Additional fees may apply for international candidates.
