Founded in 2025 by the Institute for Healthcare Improvement, this national initiative brings together hospitals and health systems to eliminate preventable harm to patients and the health care workforce. Through shared learning, transparent data, and collective expertise, it drives measurable improvements in safety across adult health care.

Driving System-Wide Change in Patient and Workforce Safety

Inspired by a successful pediatric safety initiative, this effort brings a proven collaborative model to adult health care, grounded in the belief that safety should never be a competitive advantage. Hospitals and health systems participating in the network commit to transparent data sharing, collective learning, and a shared purpose to reduce harm and build a stronger culture of safety. As Dr. Don Berwick notes, “It is high time for a major safety reboot… IHI’s new network, Transforming Safety Together, is just that reboot, and the benefits will be in countless lives saved and health preserved.”
