4Ms System-Wide Spread

Age-Friendly System-wide Spread Collaborative

IHI convened 30 US health systems over 18 months from 2024-2025 in the Age-Friendly System-wide Spread Collaborative. This learning and action community focused on embedding the 4Ms system-wide, to have an equitable impact on older adults across multiple sites and settings of care.

As of June 2026, we are thrilled to celebrate the 14 participating systems have been recognized for Excellence in System-wide Spread by demonstrating 4Ms delivery across multiple care settings: AdventHealth NC, Advocate Health, Bassett Health, CVS MinuteClinic, Greater Los Angeles VA, Hartford HealthCare, Hebrew Senior Life, Jefferson Health PA, Luminis Health, MediSys Health Network, Northwell Health, Owensboro Health, Sutter Health, and University of Utah.

All of the Collaborative systems contributed to deep learning about what it takes spread the 4Ms to varied units and settings across their organizations, and we acknowledge the contributions of the other pioneering health systems that made strides toward system-wide spread: Cedars Sinai, CommonSpirit, Grady Health, Hackensack Meridian Health, Mount Sinai Health System, Oregon Health Sciences University, Presbyterian Senior Care Network, Providence, Ralph H. Johnson VA Health Care System, Roseburg VA, UC Irvine, University of Alabama at Birmingham, and UT Health. 

The resulting resources will be made available to the public on this webpage on a rolling basis as completed. Please check back regularly!
