Gain Resources to Advance Patient Safety and Drive Improvement

The rules of engagement in patient safety have changed: patient safety progress has not kept pace with the increased complexity of healthcare operations, and what adequately enabled some progress in the past is no longer sufficient to enable it now. There is a persistent disconnect between patient safety efforts and clinical operations and traditional methods (HRO, Lean, PDSA) no longer feel sufficient in complex healthcare systems.

Beyond HRO participants will learn modern safety concepts, models, and tools best suited for driving improvement in complex operations. They will be able to employ these new skills in how they approach all elements of their safety programs. They will be equipped to adopt new safety elements, and to adapt or even abandon some old, less valuable elements.

Dr. Oren Guttman is an internationally recognized thought leader in patient safety and in the application of human factors, design science, and resiliency engineering in healthcare. He serves as the Jefferson Health System Vice President for Patient Safety at Jefferson Health, a 32-hospital system across Pennsylvania and Southern New Jersey. He holds the Edward Asplundh endowed Chief Quality and Patient Safety Officer at Jefferson Health Abington Hospital, and maintains a clinical commitment, as a Professor of Anesthesiology & Perioperative Medicine at Sidney Kimmel Medical College.

Dr Guttman is the principal architect of Jefferson Health’s Safety Management System, a pioneering framework that integrates resiliency engineering, applied human factors, and modern safety science into clinical operations. This system earned the ECRI Safety Excellence Award and serves as a foundational piece of the model for the Institute for Healthcare Improvement’s (IHI) CareOS redesign. His focus on reducing HAIs (CLABSIs) utilizing resiliency engineering principles, was among the 3 case studies that earned Jefferson Health recognition by HIMSS with the Davies Award in 2022.

Dr. Guttman’s work in integrating human factors and resiliency engineering in healthcare as a new paradigm to advance patient safety, has been featured in major media outlets including The Wall Street Journal, Becker’s Hospital Review, and Healthcare Tech Outlook. He has delivered dozens of invited presentations, and spoken on multiple podcasts and webinars hosted by IHI, AHA, CMS, and others. He serves as IHI Faculty, where he supports their Transforming Safety Together Steering Committee and co-leads their Advanced Patient Safety Course.

Leah Konwinski, MS, CPE, is a health care embedded Human Factors Engineer and the Director of Human Factors and Innovation at Corewell Health in Michigan. Leah is responsible for driving the organization’s proactive improvement strategy for patient safety, quality, and experience. She and her team specialize in leveraging human-centered systems analysis and participatory design to achieve breakthrough improvement. Their applied research centers around applying resilience engineering and human factors methods to better understand and solve for barriers to simple, satisfying, and safe work. She is internationally recognized as a deep thinker and practitioner of human factors and ergonomics, and a passionate advocate for human-centered design in the healthcare field.

Before her tenure in healthcare, Leah worked in the automotive, food production, aviation, engineering, and product design industries, bringing ergonomics and human factors insights into many design and improvement initiatives. Leah received her Bachelor of Science with Honors from the University of Guelph and her Masters of Human Kinetics with a focus on Biomechanics and Human Factors from the University of Windsor, both in Ontario, Canada. Leah is a Certified Professional Ergonomist, a member of the Human Factors and Ergonomics Society, the Resilience Engineering Association, and the Human Factors Transforming Healthcare group.

Jeremy Schwartz, MSN, RN, CNL, Director of Nursing Practice, Clinical Quality and Performance Excellence, has over a decade of nursing leadership experience. From a bedside Oncology nurse to a director of practice, Jeremy has a deep understanding of clinical operations, patient experience, nursing practice, change leadership and team development. He drives, innovates practice and implements system-wide improvements in patient safety, nursing practice, and quality. Jeremy has led successful initiatives to reduce CAUTI, CLABSI, falls with injury, improve mobility and decrease sepsis achieving significant gains in quality metrics. He designs and implements innovative workflows, mentors nursing leaders, facilitates shared governance councils, and collaborates across disciplines to enhance care delivery. He aligns nursing practice across a large hospital system.

Session Agenda

The Beyond HRO program blends expert faculty guidance with interactive learning and real-world application. Over 12 weeks, participants will explore new ways of thinking about complexity, address the persistent disconnect between safety and clinical operations, and practice applying resilience engineering principles to their organizations. Each session is designed to introduce high-level models and tools, challenge assumptions, spark fresh insights, and equip leaders with practical strategies that balance traditional methods with advanced safety science.

Continuing Education

In support of improving patient care, the Institute for Healthcare Improvement is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the health care team.

Continuing education credits for this program are still being finalized and will be updated shortly.

The Institute for Healthcare Improvement designates this live activity for a maximum of 12 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity. This activity may also be applicable for other professions that accept AMA PRA Category 1 Credits™. This course includes scheduled live components as part of the overall educational experience.

To be eligible for a continuing education certificate, attendees must complete the online evaluation within 30 days of the continuing education activity. After this period, you will be unable to receive a certificate.

Continuing education credits will not be awarded for non-educational activities, including (but not limited to) meals, breaks and receptions.

Regular Rate: $1,600 per person

Group (3 or more): $1,360 per person

This rate includes 8 virtual sessions, six optional office hours (minimum of 2 office hours required) and the use of all tools and resources.

Organizations sending three or more people can receive a 15% Group Discount. To register a group, fill out the Group Registration form.

Scholarships

IHI is pleased to offer a limited number of 15%, 25%, and 50% scholarships to assist with program registration costs for those working in:  

Independent, United States Federally Qualified Health Centers (FQHCs) that are not affiliated with a hospital or health system  
Critical Access Hospitals  
Independent practices with fewer than 20 physicians  
Hospitals with fewer than 50 beds  
Members of America's Essential Hospitals

To apply for a scholarship, please complete the online Scholarship Application by August 25, 2026. IHI will not consider applications submitted after this date. IHI will notify all applicants of their scholarship status by September 1, 2026.

All Scholarships are reviewed on an individual basis. If multiple individuals from the same organization wish to apply for a scholarship, each individual must submit an application.   

To ensure equal distribution of funds, all scholarship applications are reviewed using the same scoring criteria. All awarded amounts are final.  

Please note, scholarships are available for program registration costs only, and do not cover travel, food, or accommodation costs associated with attending a program.
