Improving Care for People with Complex Health and Social Needs

Improving care for individuals with complex health and social needs is a strategic focus for an increasing number of health systems — and public and private payers — seeking to improve patient outcomes and lower overall costs of care. Researchers have estimated that although these patients represent just 5 percent of the population, they account for 50 percent of US health care spending.

Promising models and examples exist, from which we can all learn, for redesigning care for this patient population — to better meet their needs, improve overall quality of care, and reduce costs.
