Age-Friendly Health Systems is an initiative of
The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI) in partnership with the
American Hospital Association (AHA) and the
Catholic Health Association of the United States (CHA). As of October 2022, there are over 2900 recognized Age-Friendly Health System participants.
Five pioneer health systems stepped forward to work with IHI to test ideas and learn what it takes to be an Age-Friendly Health System. The 4Ms Framework for Age-Friendly Care that emerged is both evidence-based and able to be put into practice reliably in health care settings.
