Video Transcript: FMEA

Shown here is an example of the first step in an FMEA. This FMEA is related to the addition of an intraoperative MRI. As participants identify failure modes, they are guided through the other elements, including the effect of the failure mode, and possible causes and controls that would deflect that failure mode. Each element is then scored and multiplied to generate a Risk Priority Number in the far right column. These RPNs can assist in responding to the FMEA. The highest RPNs require resolution. Lower level RPNs may be optional for action if you have limited resources.

There are many rating scales available for FMEAs; one example is shown here. Of particular importance to note in this model is that the Detectability and Preventive Measures scores are inverse. In other words, the less likely an error is to be discovered, the more risky it is. The less comprehensive a mitigating action, the more risky the failure mode, as it will proceed unabated.
