Medication reconciliation reviews may be conducted during the admission process, often by nurses on the admission unit, to identify unreconciled medications and potential errors or adverse events. This flowsheet helps nursing personnel perform a medication reconciliation process when patients are admitted to an intermediate care unit, either directly or as transfers from other inpatient care units.

 

Background

As the nurse reviews the medication orders, he or she completes a Medication Reconciliation Review Data Collection Form. The flowsheet is a tool to help the nurse determine next steps when an unreconciled medication is identified.

 

Directions

Before using this flowsheet, an organization must first implement a medication reconciliation review process with clear procedures and training for relevant personnel. This flowsheet is a sample of many possible flowsheets that may be used, once the process is in place, to assist nursing personnel in conducting medication reviews. It is recommended that the organization regard this flowsheet as a sample only, reviewing each step and making modifications to the flowsheet as appropriate for the needs of the individual organization and nursing unit.
