This tool is completed by a nurse, pharmacist, technician, or other care provider upon patient admission to the hospital. Once completed, it is verified against office records, retail pharmacies, and other data sources. At that point, the form is presented to the admitting physician for a decision as to whether to continue the drug, discontinue the drug, or modify the order. This form replaces all other medication history tools in the medical record.
Background
This tool was first used at Fairview Health Services as part of a test of medication reconciliation by pharmacy technicians. Technicians were assigned to the admitting department to take medication histories for patients scheduled for surgery. This reconciliation project reduced the number of defects from 1.5 per patient to less than 0.2 per patient.
