Due to limited availability of pharmacy resources, Moses Cone developed a "blended model" for medication reconciliation where pharmacy is consulted for specific criteria (i.e., more than 10 medications, HIV medications, anticoagulant medications, or antiseizure medications). The criteria for the pharmacy consult appears in the shaded area of the form ("Write Pharmacy consult for Med Rec if patient is taking"), at the end of the medications list. This process works well to limit pharmacist involvement in medication reconciliation, based on predefined criteria, to only those patients who will most benefit.
