The Health and Safety Passport is designed to help patients take an active role in their medical care by keeping track of their health history, medications, health visits, emergency contacts, etc. This tool can provide families and health professionals with critical information in a moment's notice, and can be used by providers as a medication reconciliation tool.

 

Background

This tool was developed by the medical center's Marketing Department in association with the Center for Patient and Community Education as a way to create a portable health record for patients and families coming to the medical center for their care.
