Health Disparities Collaboratives Training Manual for Chronic Conditions

This training manual, consisting of four parts, was developed to help health centers and other primary care organizations improve chronic care. In addition to describing three models that may be used to improve care people with chronic conditions, the manuals also contains specific changes, resources, and tools for diabetes, asthma, and depression.

Background
The manuals were originally developed as a training and spread tool for the Health Resources and Services Administration Bureau of Primary Health Care to support the national quality improvement initiative, Health Disparities Collaboratives. The manuals were published in April 2002.
