This data collection tool is used to monitor whether providers practice contact precautions (i.e., use of gown, gloves, and mask) and hand hygiene (i.e., use of alcohol sanitizer or soap and water to decontaminate hands) with patients in isolation. The Contact Precautions Monitoring Tool may used in conjunction with the Hand Hygiene Monitoring Tool.

Write in the name of observer.
Check whether the employee performed hand hygiene and observed contact precautions [use of gown, gloves, and mask (optional)].
Write in employee profession.
At the end of the observations, write in the time observations are ended.
