Collect Allergy Information on Admission

Lack of current allergy information can lead to the administration of a medication to which a patient is allergic. Be sure to collect allergy information at the moment of admission in all settings — inpatient, outpatient, and ambulatory surgery — so it can go on record before staff members order or administer any medications.

Tips

Record information immediately and make sure it is visible to anyone who may order or administer medications.
Make sure the information is transmitted to the pharmacy as soon as possible.
Place allergy information in multiple locations.
Ensure that the allergy information follows patients
Use information from previous admissions if the patient is unable to provide it upon this one.
