After a powerful conversation during last week’s Institute for Healthcare Improvement (IHI) Community Roundtable on Responding to Medical Errors with Care, Not Criminalization, IHI was asked by our community to return for a discussion specific to just culture, psychological safety, and clarity around the call to action for health system leaders.

IHI President and Chief Executive Officer Kedar Mate, MD, was joined by a panel of industry leaders for a second free virtual roundtable to address these urgent issues on Thursday, April 21 from 4:00 – 5:00 PM ET.

In the spirit of community and improvement, IHI hosted an expert panel of industry leaders for a complimentary, virtual roundtable-style discussion on Friday, April 8, 2022, from 12:00 PM – 1:00 PM ET.

  • Provided key next steps for leaders, providers, and patients
  • Offered recommendations rooted in the concept of "just culture" with key emphasis on system-level accountability and learning
