This two-and-a-half-day program (November 2-4, 2015 in Wellesley, MA) will help your organization effectively manage multiple change efforts, such as:
  • Evolving data systems
  • Optimizing physician partners
  • Coordinating care
  • Attaining payment incentives
  • Developing community partners

Building on IHI’s years of population health experience, expert faculty will help you and your team accelerate efforts to improve care and outcomes for your patient population, while achieving financial vitality and ensuring sustainability. Join us and learn innovative approaches to leveraging financial models, integrating data systems, developing improvement infrastructure, leading change, and optimizing partnerships with patients, providers, and communities. Learn real world lessons from a variety of organizations that are delivering population-based care. You’ll also have the chance to learn from other participants, who may be facing similar challenges, as they work to transform care.

During this two-and-a-half-day program, you’ll collaborate with your team to develop an action plan to implement upon your return home. This seminar includes a virtual component so members of your extended team at home can participate in the sessions most important to them.​​​​​​​

This program is ideal for teams from new (or evolving) ACOs who are working on population management in an effort to achieve the best outcomes and most cost-effective care.
