9:30–10:30 AM​​​ Paradigm Shift: Moving from the Traditional Doctor’s Office to Team-Based Care
Participants will engage in group discussion to debrief both the patient and provider perspectives. We will also introduce the concept of Population Health, and ‘why’ the importance of Team-Based Care.  After this session, participants will be able to:
  • Gain a better understanding of why now is the time to invest in team-based care
  • Understand the broader conceptual framework for integrating population health into primary care
  • Describe reasons why making the shift may be beneficial to their organization
​10:30–11:15 AM Creating a Team-Based Culture and Shared Purpose:
Working With the Change Concepts of Team-Based Care
How do we move to a team culture? What role does leadership have in this transition? Goal-based planning, performance improvement, and patient-centered principles form the core of this important organizational shift. After this session, participants will be able to:
  • Talk about leadership culture and how it facilitates the transition to team-based care
  • Define how to move toward a performance improvement infrastructure
  • Integrate tools for how to include patients as part of the team

11:30 AM – 12:30 PM​ Self-Assessment and Goal Setting
Participants will utilize their results from the prework to start to build their teams and prioritize projects around the work that needs to get done. After this session, participants will be able to:
  • Identify potential gaps in areas of focus
  • Outline and develop next steps towards team based care approach
12:30–1:30 PM​ Lunch: Topic Tables
1:30–2:15 PM​ Understanding Your Population and Building a Team to Match 
Group activity with personal patient.
​2:15–3:00 PM​ Deep Dive Into Roles and Functions
What is the potential function of each team member.  Defining the roles and responsibilities.  What other functions are needed to manage a panel of patients.
3:00–4:15 PM​​​ New Team-Based Primary Care Paradigm Exercise
Teams will re-do the previous exercise to test their new understanding of how to optimize the roles of each core care team member.​
​4:15–4:30 PM Setting the stage for Day 2 Financial Model Discussion
Overview of the financial model.
​4:30-5:00 PM ​Influencing and Embracing Change
Exercise on being open to change and new ideas, and ongoing incremental change.

This session will explore how an organization can successfully transition to a team-based care service model.  What are the financial models that support this style of care and how to navigate the upcoming changes in Medicare and Medicaid payments.

10:00–11:00 PM​​ Picture of High Performing Teams: Application in Primary Care
This session will discuss the behavioral health skills teams need, and other aspects of high performing teams.
​11:00 AM–12:00 PM Using Data to Proactively Organize the Day, Eliminate Future Work, and Drive Improvement

12:45–2:15 PM​ Daily Teamwork Functions
What a workflow is and how to put one together.  What are the critical workflows that need to be part of team based care.

How to structure a colocation room. Intentional planning to create good team environment.
