About IHI Collaboratives


IHI’s most intens​ive front-line improvement work happens in Collaboratives, nine to twelve-month programs designed for organizations committed to achieving sustainable change within a specific topic area. 
 

Fees & Enrollment

Enrollment Fee

The cost for one year of participation is $20,000. Interested organizations are encouraged to partner with health care, health plan, and community participants. A limited number of partial scholarships is available for safety net organizations.

Special Discount

A reduced rate of $10,000 per team applies to the following:

  • Federally-qualified health centers
  • Safety net organizations
Group Discount
A group discount of $17,000 is available for organizations enrolling 3 or more sites.

If your system or coalition is interested in enrolling in the Better Health and Lower Costs for Patients with Complex Needs Collaborative, please e-mail BetterHealthLowerCosts@ihi.org. We welcome new teams to join the Collaborative through September 2015.
 
New teams and new team members will prepare for the work of the Collaborative by focusing on the following activities: set-up, prework, and learning about the Triple Aim framework under which this work resides. Structured pre-work guidance will be provided and a series of calls will support the new team preparation. The BHLC team and faculty are committed to helping new teams complete this prework in advance of the Collaborative kick-off call in September. 
 
During the enrollment process, you will be asked to include the name and contact information for the roles below. Please have this information available before beginning registration. Organizations can send up to four participants to the face-to-face meeting.
  • Your Key Contact is the “captain” of your organization’s Better Health and Lower Costs for Patients with Complex Needs participation and is often also the Team Leader.
  • Your Team Leader may also be the key contact, or may be another individual who will guide the team on good project management, idea generation, testing, and implementation. This individual will also meet regularly with the organization Sponsor to review progress and results.
  • Your Sponsor is an organizational leader who is responsible for and accountable to the organization for the performance and results of the Better Health and Lower Costs for Patients with Complex Needs Collaborative team. This person is not a member of the team, but is responsible for securing the resources that are required and for communicating about progress to others in the organization.
