Health and Well-Being Measurement Approach and Assessment Guide
This guide is a resource for understanding health and well-being and administering the Well-Being Assessments developed by the 100 Million Healthier Lives Metrics Team.
Adult Well-Being Assessment (12-item)
This assessment, designed for administration to individuals ages 18 and older, gives individuals and groups working to improve the health of their communities a short, yet holistic, tool to measure and track improvements in health, well-being, and equity. The brief health and well-being questionnaire, comprising items from validated instruments, was created in collaboration with experts in the field and tested in member organizations and communities. The questionnaire may also include identifiers for key demographics to enable stratification of the results to assess equity in health and well-being.
Adult Well-Being Assessment (24-item)
This assessment, developed in collaboration between 100MLives Metrics Team and Harvard Human Flourishing project, is designed for administration to individuals ages 18 and older. It gives individuals and groups working to improve the health of their communities a comprehensive set of measures for health, well-being, and equity. The 24-item assessment addresses more aspects and domains of well-being than the 12-item version and comprises items from validated instruments. The questionnaire may be used in its entirety or in sub-sets. The questionnaire may also include identifiers for key demographics to enable stratification of the results to assess equity in health and well-being.
Youth Well-Being Assessment (12-item)

Well-Being in the Nation (WIN) Measurement Framework
The WIN Measurement Framework offers a set of common measures to assess and improve population and community health and well-being across sectors. The framework is divided into three elements: core measures, leading indicators, and a full flexible set of measures.
