MyStory is an interprofessional effort designed to personalize the care provided to pediatric patients and transform the hospital experience for the patients and families by capturing each pediatric patient’s “story” and engaging the child in decision making. Included are templates for infants, toddlers, school age children, and teens to share information about themselves and their caregivers.
The goals of MyStory are to:
- Document the patient’s “story” within the electronic health record
- Use the patient’s “story” to personalize the care provided, and involve them in care planning and decision making
- Implement the intervention involving all interprofessional care team members who care for pediatric inpatients
- Measure the impact of the intervention using patient satisfaction data
