This tool is designed to help patients remember why, when, where, and how to take their medicine(s). Ideally, the medication log should be filled out by the patient and provider together during a patient visit, and reviewed at each subsequent visit.
This tool was developed by the Cardiovascular and Public Health Detailing Programs for use in the Cholesterol Action Kit, one of many “Detailing Action Kits” from the DOHMH. The kits contain clinical tools, resources for providers, and patient education materials to promote evidence-based best practices.

A Spanish version of the tool is also available for download from the DOHMH website.
