An individual’s ability to access high-quality health care and live a long and healthy life is largely determined by where they live and how much they earn. In communities across the country, however, pioneering organizations are working hard to achieve a more equitable health system. This article summarizes key findings from the latest research on health disparities, a joint effort between IHI and the Commonwealth Fund, and provides two examples of how innovation in the delivery of care is reducing these disparities.
