Lessons Learned from Two "Super-Utilizers"

In this storyboard presented at the 25th Annual IHI National Forum in December 2013, University Hospitals Case Medical Center analyzed their experience caring for two patients considered "super-utilizers," who had total charges of $600,000 in one year. After implementing several interventions, including building capacity for primary care teams to customize patient care plans, the total annual charges decreased to $3,000.
