The use of "triggers,” or clues, to identify AEs is an effective method for measuring the overall level of harm in a hospital. The IHI Global Trigger Tool for Measuring Adverse Events is a tested, proven, and sensitive tool to measure harm. It provides instructions for conducting retrospective reviews of adult inpatient records using triggers to identify possible AEs. This tool includes a list of triggers and instructions for measuring the number and degree of harmful events, and, in a recent study, detected ten times more confirmed, serious events than other methods.*
The Web&ACTION: Using the IHI Global Trigger Tool for Measuring Adverse Events will teach participants how to use this tool to identify patient harm and how to use these results to measure the effectiveness of safety improvements designed to reduce patient harm. Faculty for this program includes the developers of the original IHI Global Trigger Tool. These faculty have refined the IHI Global Trigger Tool over time, based on experience in both using the tool and training thousands of reviewers.
The program is designed for teams of reviewers who intend to use the IHI Global Trigger Tool in their own hospitals. All members of a designated review team should participate in the three webinars — on February 14, February 28, and March 14. Each Web&ACTION registration includes one web/phone connection but unlimited attendees per connection. This program also includes three additional telephone calls designed, in combination with the three webinars, to guide the beginner IHI Global Trigger Tool user into a sophisticated user able to create a system of tracking harm and improvement within their organization.
