This program is ideal for change agents from around the world in health-related organizations who are responsible for developing strategy, delivering front-line care, or crafting policy and supports for a specific population.
The seminar is ideal for individuals working in the following types of organizations:
- Integrated health delivery systems;
- Community clinics and safety net systems;
- Regional health districts or systems;
- Physician groups;
- Health plans or plan sponsors such as employers;
- Regional coalitions, and public health departments;
- Social service agencies and organizations;
- Government agencies and state, county, or municipality initiatives; and
- Any organization working on system transformation efforts such as Patient-Centered Medical Home implementation, community needs assessment and collaboration, or accountable care strategy.
Teams of three to five people, from an organization or region, are encouraged to attend. Individuals are welcome, although experience has shown that Triple Aim implementation is dependent on the collaborative effort of leaders and strong program involvement.
