In this rapidly changing health care environment, now is the time for primary care practices to rethink their approaches to providing care. Doing things “the way we’ve always done it” is no longer enough; primary care organizations must learn how to transform their systems of care in a way that will lead to better, sustainable results.  Meaningful use, the patient-centered medical home, advanced primary care, patient- and family-centered care, and electronic health records all constitute new challenges for office practices, yet they also present new opportunities. When equipped with foundational skills and knowledge, organizations that have a will to change and improve can thrive in this new environment.
 
To help primary care practices adapt and succeed, the Institute for Healthcare Improvement (IHI) is pleased to offer an intensive program, Transforming the Primary Care Practice. Designed to bring together the best knowledge available, this three-day intensive program will guide participants towards exceptional advanced primary care and the meaningful use of information technology. A group of expert faculty will teach participants accessible, continuous, and coordinated family-centered care using a team approach, health information technology, and shared decision making. This program will lay the foundation for ongoing improvement, while also providing the opportunity for practices to immerse themselves in key changes that will propel them towards a system that works better, is more cost effective, and meets the needs of the community.

By the end of this three-day intensive program you will be able to:

  • Describe and carry out key changes that lead to advanced primary care and can result in certification and recognition for the patient-centered medical home

  • Recognize that crucial care processes and systems should drive the design of clinical information systems so that your electronic health record works for the patient, the practice, and satisfies meaningful use criteria

  • Identify strengths and opportunities for improvement in the delivery of primary care services and the development of partnerships with patients and families within your practice

  • Apply principles from the science of improvement to accelerate progress toward your organizational aim

  • Develop pragmatic ideas for change to achieve your goals and objectives

  • Identify priorities and develop a plan and timeline to achieve fundamentally improved performance levels: better clinical outcomes, lower costs, exceptional patient and family experience, and improved efficiency and access

  • Identify ongoing learning opportunities and collaboration that will be helpful to sustain and promote accelerated progress toward organizational transformation
