According to health care attorneys, planning for serious clinical adverse events is a component of most risk management plans, yet an estimated 20 to 30 percent of providers still do not have an effective plan in place for responding to such an event. This article discusses efforts to support the need for a comprehensive plan to proactively manage medical mistakes, underpinned by creating a culture of respect and accountability. IHI's white paper, Respectful Management of Serious Clinical Adverse Events, is noted as a comprehensive resource to support such efforts.
