The IHI 5 Million Lives Campaign’s national learning network is made up of committed professionals, like you, in thousands of organizations across the country. Why not connect with them? Have you ever wished you could consult with a rural hospital similar to yours that has had success with medication reconciliation? Reach out to a Mentor Hospital. Looking for local support with your improvement efforts? Contact your state Node.

Use the interactive map below to join in the shared learning that’s at the heart of the Campaign. Here’s how:

• Click on any red marker to see information about hospitals enrolled in the Campaign.
• Click on a blue marker to learn about Mentor Hospital results and get their contact information.
• Click on a green marker to see Node contact information.
• Type in the Search box at the top to find any hospital enrolled in the Campaign.
• Click on the map and drag to look at different parts of the country.
• Use the button in the upper left corner to zoom in and out.
• Click on the filter link to view only Nodes, or mentors for a specific intervention.
