I would like to utilize the SBAR technique and documentation tool at our hospital. I would like to hear from other hospitals and how they are utilizing the tool, what modifications were needed, outcomes and benefits. Thank you

We are also exploring use of this tool. Just wondering if others have had experience with it and what your thoughts/recommendations might be. Thanks.

We have created notepads with the SBAR format to help nurses begin to think and organize their thoughts before calling physicians. It has been very helpful

We have revised the SBAR tool slightly to use in the home health setting. I was wondering if any other organizations are using the tool in home health.

After multiple database searches, I have been unsuccessful in finding any published research on the effectiveness of the SBAR tool itself. Is anyone aware of any research publications?

We are in the process of developing SBAR for my SICU. I am not finding many SBAR tools to use as examples for my chief of surgery. Any examples would be welcome.

Thank you in advance for your help.

Carol

I would like to know how others have addressed the new NPSG related to communication at the point of hand-off in care, and the need to implement a process including the ability to ask/ respond to questions. If you have used the SBAR model and added the question component, how have you done that? Thanks.

Has any acute care facility expanded the form?

Dorothy,

We are just beginning to put together a RRT, could you e-mail me an example of the tool you are using that works best for your nurses?

Thanks,
