I attended the Kaiser National Quality Conference in June, 2005. At that time, Dr. Roger Resar mentioned that work was underway to develop a trigger tool for use in ambulatory care and that it should be ready to test in November. Any word on progress? Thanks.

We are currently testing the tool with 5 organizaions across the country. We are a few months behind our timetable, but hope to have a exportable tool in 3-4 months. KP is one of the organizations that is extensively testing the tool. Roger

I am looking for the "Practice Charts" and Answers to the practice charts" on the website. Are these available? That was my understanding from the power point presentation held on March 20, am I correct?

The Training Record Set for the IHI Global Trigger Tool is available on IHI's website at:

Our hospital has recently began using the global IHI trigger tool. I would like to know if anybody has an established process for using the trigger tool and would be willing to discuss their process with us.

We started an IHI Trigger Tool User Group that may be of interest. Many people around the world are using these tools to measure adverse events in their organizations. Trigger tool users often express interest in connecting with other users to share ideas and strategies for effective usage – such as interpretation, analysis and presentation of the data.

Through the use of an email listserve, IHI supports the User Group for these types of discussions. The main purpose will be for users to connect with each other. IHI will maintain and moderate the listserve as needed and expert faculty on trigger tools may participate in answering questions on occasion.

If you are interested in joining the IHI Trigger Tool User Group please send a blank email to Listmanager@ls.ihi.org and in the subject put “subscribe triggertools” (without quotes).

After you receive your confirmation, you can then post your questions by sending an email to triggertools@ls.ihi.org
You will likely receive responses from other users of these tools

Dear IHI
I need advice on coding of triggers. Should we code elective triggers e.g elective admission to ICU post CABG, premedication and post op anti-emetics for vomiting. elective readmission within 30 days. Appreciate your input to questions.
