I am seeking comments from other hospitals that have initiated a daily goal sheet. Can you provide a list of contacts?

Hi Regina --

You may want to check out the 100,000 Lives Campaign Mentor Hospital List that provides information (including contact details) for hospitals that have offered to serve as mentors to other organizations re: implementing the six Campaign interventions. Many of the hospitals on that list note that they are using daily goals with their patients (see specifically the sections on Prevent Central Line Infection and Prevent Ventilator-Associated Pneumonia).

The searchable Mentor Hospital list is available at:

I am looking to design a daily goals worksheet for our critical care unit and have so many ideas but don't know where to start. Can anyone help with some ideas of some examples to view. We do rounds with multidisciplinary members but are looking to incorporate a central line bundle.

Thanks.

At our hospital we are facing problem of long stay and long-term care of ICU patinets. We dont have concept of nursing homes in our country. What shall we do to address the above mentioned issues and how should we communicate to the families and convince thenm to take patient home as they dont have good out comes. Most of the times hospital has to bear teh cost, we dont have that kind of insurance system coverage in our country and families vant afford to pay hte expenses.

Thanks

If you have a worksheet you're willing to share, I am very interested in seeing it as I am in the process of creating one. Thanks for any input.

I am currently an Rn and I am attending WVU working on my Bachelors degree. I have a evidenced based research project this semester and I have chosen open vs. restricted visitation. If anyone has a copy of the icu daily goals worksheet that I could review it would be very beneficial,as part of my research will be to implement and introduce a new way of looking at areas that could provide better patient recovery. I am excited about this project as a new nurse this gives me an opportunity to look at development and new direction.

I would be interested in hearing from someone who has implemented a daily goals worksheet and has been into it for a long period of time (a year). How did you implement, what were barriers, how did you overcome them? My email is mculbert@trover.org. thanks
