I'm wondering if nurses are documenting the information that is given via the SBAR system? The situation and backbround parts of the tool are easily understood and used, but many nurses are reluctant to use assessment and recommendation because they believe this is outside the nursing scope of practice. They don't want to chart that they recommended a method and then worry that the physician doesn't follow through with the recommendation.

I'm working on an SBAR communication tool for the RN to call the doctor when the Blood sugar is low and has been treated per hypoglycemia protocol. Anyone who has already put this together? I'm most uncertain how the script should read for the recommendation section.
For sure I would have them recom change in Insulin regimen, esp. if using only Sliding Scale Regular ac hs, which is common at our hospital.

What I've read about SBAR is it's only a communication tool. The nurse doesn't have to document anything except the change of patient's condition, the time of notification and the intervention.

SBAR is a great communication framework. Most of the examples for its use seem to be from Nurse to Physician. Has anyone used this and developed any Nurse-to-Nurse patient transfer or communication checklists in an SBAR format?
