I had a problem in reporting medication error from the staff. A series of lectures was done to explain the importance of reporting errors .

I would like to know if there is a method in which mediction errors in the ER can be detected even if they are not being self reported. Iam looking for a method to improve the reporting or evaluation of the med. errors in ER.

I do not know of any method specific to the ED.
As with any medication error review, one uses observation, review of orders and self-reporting.
Sorry, I do not have an easy method.

I agree with Frank that observation may be the best in the ED. On inpatient units automation can help with some of this such as dispensing machines and bar coding, however these are not always practical solutions for the ED.
