We have had great discussion regarding the use of H2 Blockers and PPI's for the PUD Prophylaxis. We know that the bundle advocates the use of H2 Blockers only for PUD prophylaxis - but we are getting some push back from our physicians - especially since PROJECT IMPACT from the Society of Critical Care Medicine advocates the use of both. Thoughts?

What are you using for ulcer prophylaxis in your facility? Do you use only IV or something per feeding tube?

I am wondering about the statement in the first paragraph of this section that reads, "decreasing the pH of gastric contents may protect against a greater pulmonary inflammatory response to aspiration of gastrointestinal contents."
This statement argues against the use of H2 receptor blockers in the ICU. Is it a typographical error?

Teresa - great question and no, it is not an error. This element of the ventilator bundle, as with the others has risks and benefits. We have tried to include all the relevant information. While the goal is provide PUD prophylaxis to vented patients, one must also know the risks of doing so and of the various choices to make the right decision for the patient.

Yes, I believe it is an error. This statement contradicts the erroneous statement: "The effects of aspirating acidic contents may be worse than those with a higher pH."
