IT IS HARD TO DO. But it's important to be implemented together with all other components of the bundle for VAP prevention.
Now, we are doing MultiDisciplinaryRounds and was attributed to one or two nurses to check the bundle list each 8 hours.

Hi,
Question:
Are there any other tools out there for measuring HOB elevation other than using the gauges on the side of the Total Care Hill-Rom beds, the lower side gauge near the bottom side of the Hill-Rom Advance beds, the foot of the bed with the Stryker bed by pressing fowlers postion for a digital reading? I have heard that Home Depot has a digital inclination meter for $19.99. Also I know that Hill-Rom has software available with reminders that could also collect stats, monitor HOB in mins or hours could be added for a fee of $200. on each of the beds but I believe would be extremely expensive.
Any other ideas out there.

Hi Mary,
I have seen some places uses very low-tech approaches for head of the bed, for example:

- a poster on the wall of the room showing the correct position of the bed so that a caregiver standing on the opposite side can visually see if the bed is in the right place

- visual markers on the wall behind the head of the bed that can only been seen (or not seen) if the bed is too low or too high - "if you can see this the bed is too low"

You might also try contacting some of our VAP mentor hospitals, listed on the Campaign section of the IHI website. I'll bet there are other great ideas out there!

We recently became vigorous in enforcing 30 degrees (not 20 not 25) and since doing so have seen a dramatic increase in our pressure ulcers. It seems that the VAP rules conflict with the decub prevention guidelines. Anyone else notice something similar when instituting the 30 degree HOB elevation rule?

I am new in this job but was curious. What medications contraindicate raising the HOB. I am trying to research this and have not found my answer yet.

A low tech, inexpensive tool is to use a level, which is available from your local hardware store. Attach the level to the side rail, with the top end of the level elevated (0.6 inch higher per inch of length) or 1.8 inches for a 3 inch long level.
