We presently use a form similar to this but with additional features such as type of reaction to allergies listed. We also have several additional columns for discharge orders such as last dose date and time given, script given. A physician has to sign, date and time. I like the separate area for new medications to take following this visit. My question is would this form be appropriate for series patients such as in physical therapy, cardiac rehab, etc.?

One of the things that you can do in the serial outpatient setting is to set up a policy for how often the med list needs to be updated as it may not be necessary (and burdensome) to do this every time.
