Do any community hospitals have a medication reconciliation form that they have used successfully within their institutions when a patient is transferred from floor to floor or floor to OR? Also, do you have a corresponding policy that you would be willing to share?

We are just starting the process of setting up a system to reconcile medications at all points of care. My area of concern is patient transfer- so I am also interested in success stories, tools, and tips.

I am at a community hospital that uses Meditech OE and we do have a Medication Transfer Order form which is generated off of our computerized medication order entry via the pharmacy module. We also have a policy that coincides with the process. The form is used for all level of care transfers including to the OR and from PACU to all inpatient units. We have been very successful in eliminating duplicate medication order and missed medications. Additionally, it has reduced the amount of time required to decipher transfer orders by nursing personnel.
