For LOS for the trigger tool, should we calculate this by counting the hours pt is in the hospital or the calendar days?

Hi Allison,
The LOS is days not hours so it is best to keep it simple and just subtract admission date from discharge date.

Is there a formal training program for new reviewers or was everyone self-taught (using the guide)?

Some people have used the guide for self-teaching. Many have attended our formal training programs. We always do a session at the IHI Forum, if you shall be attending that. Periodically we do web-based conference calls although none are on the schedule at the moment as we recently completed one.
There is a recording of a call that Roger Resar and I did on GTT during the 5 Million Lives Campaign. It can be found on the IHI website - under Programs, Campaign and then the Materials tab. Scroll way dow the page to section on IHI Global Trigger Tool. There is a recording there.
We also have a free Trigger Tool listserve. To join:
1. Send a completely blank email (no subject, signature, or text in message body) to: subscribe-triggertools@ls.ihi.org.
2. You will receive a confirmation message.
3. Post messages to the listserv by sending emails to triggertools@ls.ihi.org.
It's a great way to stay connected with others and post questions to those experienced in this.

You are most welcome! We do not have another offering scheduled at this time. Next one would be in 2010 at earliest. It would appear under Programs on the IHI website - under Audio and Web programs.
