This is excellent intuitive, and quite useful. However the only non-intuitive piece is the meaning of "pass substitution test", and having it defined would make it even more useful.

Our patient safety committee liked this tool, but would like to know what "Pass substitution test" means.

Substitution Test per Jim Reason:
Would 3 other individuals with similar experience and in a similar situation and environment act in the same manner as the person being evaluated. If the answer is yes - the problem is not the individuals, but more likely the environment that would lead most individuals to that action.

If the answer is No - that other similarly experienced individuals would not have acted in a similar manner, it's more likely that the individual being evaluated is more culpable/accountable and in need of something - whether is be counseling or removal or whatever.
