IS IT RECCOMMENDED TO PLACE THE CHECK LIST AND/OR COMPLIANCE RECORD IN THE PATIENT'S PERMANENT RECORD?

That depends on how you use the checklist. In some cases, checklists are part of the permanent record - for example, pre-operative checklists. Checklists are also used as tools for audit, quality assurance and improvement and then are not part of the medical record; in those cases, they are only serving as data tools.
You need to decide at your facility how you are going to use the checklist. If it will be part of the permanent medical record, you will need to have one for every patient who has a central line. As your work progresses, if you intend to move towards sampling (not completing for every patient) just to have data, then it should not be in the record.
Your Medical Records and Risk Management Departments may have some advice on the matter.

Corner case: How should compliance be measured if a patient has 2 different central lines and one of the central lines met all bundle measures, but the other central line did not. So, in summary, same patient, 2 central lines, one line 100% compliant and other not. Is it measured as 2 patients or is that one patient 0% compliant? Tough one, huh?

Regarding documentation for continuance of the line/daily assessment - Is RN documentation of the daily need appropriate? (assuming discussion w/ the MD) or does the MD have to document in order to be compliant with the bundle?
