Continuation of all home medications done based on the reconciliation form frequently leads to adverse consequences beacuse of disregard for the patient's acute medical problem-- eg continuation of ace inh in patient admitted with hyperkalemia, continuation of antihypertensives in patients with dehydration and borderline BP, metformin continuation in pts whose renal function has worsened due to acute illness etc. Having said that this is not a problem of the reconciliation process per se but an error of physician judgement.

Our hospital is struggling with developing the best form for nurses to complete the medication reconcilation process. We have tried several without a great success in compliance related to steps involved.

Can anyone share their medication reconciliation form with me? Thanks

There are several examples of forms that hospitals have used for reconciliation on our website - look under Patient Safety, Medication and Tools.
