As a Medication Safety & Quality Project Officer for Clinical Services Directorate within the South West Area Health Service, I have just completed the first trial phase of our project focussing on medication safety. using an adapted version of this tool.
The general concept has been positively received with 100% clinicians at one of the trial sites recommending the briefings continue.Raising staff "alertness" and promoting a "safe culture" is certainly challenging as is sustaining the use of the briefings without the ongoing guidance/driving of a manager position. Onward we go!

I would be interested in hearing about success stories in instituting patient safety briefings. I am just starting this initiative and could use the help.
