Patient- and Family-Centered Care--A Partnership of Extraordinary Potential

Welcome! IHI enthusiastically supports patient- and family-centered care, and we are working with and learning from patients and families in myriad ways. In reviving this discussion group, we hope to offer another avenue for discussion where people can share and learn from each other.

Among the questions that continually arise about this topic are the availability of resources, evidence of its impact on health outcomes, organizational and operational best practices, and models to effectively support implementation and sustain patient- and family-centered efforts. Building on IHI's work over the years, recent focus has included assembling a community of expert organizations and individuals, identifying key published resources, locating organizations who are exemplars, and planning for an expert panel and white paper.

What we are finding is powerful, as noted in the recent IHI online article entitled "Delivering Great Care: Engaging Patients and Families as Partners." Within the IHI 100,000 Lives Campaign, specific materials are available for patients and families and we are continuously seeking ways to include patients and families in our work. We invite you to share your experiences or ideas in this discussion group, or to ask questions of other colleagues who are working to provide more patient- and family-centered care.

Thank you for providing resources and ideas on this important topic. Our organization is embracing the concepts of family centered care, and the inclusion of patients as partners. We will soon be adding patients to some of our committee's including our patient safety committee.

We are looking for any criteria and/or orientation materials, especially for use in the area of patient safety and patient safety committees.

Please continue your work and support of this topic, it is greatly needed, our patients and families need you to be their voice.

We have been involved with this concept for 10 years in our Joint Center, affectionately known by the patients as "Joint Camp". Family members serve as "coach" , receive a "coach" button, attend preop classes, help out with therapy which we provide in a group, receive post op transition instructions with other coaches, take a test called "coaches checklist", and return one month postoperatively to our "patient-coach" luncheon where we encourage them to tell us how we can be better. If the patient has no family we provide a volunteer to serve in this role. As one nurse recently said ,"I used to cringe when the family appeared, now I cringe if they don't". Seven thousand patients later we've had a few spats but no coaches have been fired. In fact at our annual reunion we have awarded "coach of the year" prizes to those coaches getting the most praise (in writing) from their patient. It's fabulous!!!!!!!!!!! Thanks for bringing attention to this.

Thanks for the first post! The Dana-Farber Cancer Institute has had patient and family advisory councils for 9+ years and patients specifically sit on the Patient Safety Committee. Two people who can help are Deborah_hoffman@dfci.harvard.edu and saul_weingart@dfci.harvard.edu. There are also articles about the DFCI experience on the IHI web site. Send me an email directly if you are having any problems and I can attach them. I have seen other articles. Lets see if we can hunt them down.

Thank you for bringing alive the concept of pt/family centered care. It is a concept I've had in my heart for years, but didn't know how to go about making it happen. You've provided some stepping stones for me personally and for the organization to begin making this type of care reality. I will be bringing this concept up in a committee tomorrow and your website has been a great resource.

I'm hoping people will become active on this discussion thread to exchange ideas on individualizing this concept to each organization.

Thanks for making your expertise available. We are looking for models of bedside report tools from any and all who are currently using them in practice in a Med-Surg setting.
