At the end of the webinar it was mentioned that Titles could post their QM Plans to share with others or could send them to the Center for review/suggestions. I have not been able to find where these have been posted or the e-mail address to send the plan for review. Would you please reply to this post with that information?
Also, please post the results of the webinar survey questions.

I am also wondering where to look for any posted QM Plans. We would like to see documents prepared for Title I EMAs particularly.

You can send the National Quality Center your Quality Management Plan if you would like to share it with others. Please send to NQCTA@health.state.ny.us.
If you would like to have your plan reviewed for suggestions, please fill out a TA Request Form available at http://www.ihi.org/IHI/Topics/HIVAIDS/TheNationalQualityCenterNQC.htm and email it to the address above.
