Large percentage of patients in acute care, especially in community hospitals, have been seen multiple times in the course of a year, and chronic conditions, medications and social systems do not change much. Find a way to have the computerized intitial assessment repopulate fields that probably have not changed (ie, medications, chronic conditions, past surgeries and medical history) and have the patient/family review for changes.
