We have developed our own version of a medication reconciliation form. We have gone hospital wide with the process as of Nov. 1st. We are having resistance from some of our physicians, surgeons mostly, about signing off on the admission portion of the form. They do not want to sign the form at admission to give the nurse clarification of which home meds to continue or discontinue during their hospitalization. Are any other organizations having this problem? If so, what was your solution? I would appreciate any comments. Thanks

A few questions....
What type of form are you using? Is this form additional to the order sheet? Some places have had success by using a medication form that also serves as an order sheet. There are examples on our web site.
Where are medications being ordered and by whom?

Currently we have two different forms, one for reconciliation and one for orders. Sometimes, if a patient is on too many medications, it makes very difficult to fill up all these forms. It is a great idea to come with the order form as a med reconciliation form which does not affect the patient safety.
