Please help us clarify some confusion with the definition of denominator in the percent of unreconciled medications. Here is an example: A child with cystic fibrosis is admitted to the hospital with a home medication list of 10 medications. During admission reconciliation, the physician makes a decision to only order 5 of the home meds and documents the other 5 to be discontinued. The physician also additionally writes orders for 3 more new medications at the same time. What is the denominator in this example, 10 or 13?
