I am a student looking for background on why medication reconciliation is an important quality improvement. If anyone has information regarding studies showing reduced error/improved patient outcomes in relation to medication reconciliation, please email me at ecburges@indiana.edu.

Medication reconciliation (to prevent adverse drug events) is one of the key interventions of IHI's 100,000 Lives Campaign.
There is an annotated bibliography available as part of the Campaign materials posted on IHI.org that may be useful to you.

Responding to both of the prior questions:
1- Can midwives sign medication reconciliation forms? That is up to you and depends on the process you use for reconciliation. There is no requirement to use a form; many places find it helpful but not all. If the form is supposed to serve as an order form, then does the signature activate the orders? If so, does your medical staff allow midwives to write orders?
2- Regarding measurement of med rec on admission, you can use any of the med rec measures we note under the Patient Safety Medication Systems measures or in the 100k Campaign Getting Started Kit for this topic. An easy measure is to sample some admission records and look to see how many meds needed to be addressed (sum of meds being taken before admission plus new ones added in admission orders) and determine what percent were not reconciled.

How does one document that this home medication information may be incomplete or inaccurate?
In other words, is anyone using a "disclaimer"?
As the home medication information is most times dependent upon the patient or responsibile party's oral history and the patient is to receive a copy of their medication reconcilitation record upon discharge.
