I'm looking for a policy on medication reconciliation for the patient seen in outpatients services. All policies I've seen address inpatient /outpatients who convert to inpatient. Thanks georgia

Hello - I am working on a medicines management programme in England where we are encouraging medicines reconciliation at transition points in secondary care.
Does anyone have any experience of using this process at transition points in Primary Care? In particular between Community Pharmacy and General Practice or residential care homes?

I understand this to mean that reconciliation starts at admission, or point of entry, not to be done in ER triage for a patient that will not be admitted. What are emergency departments doing?

Reconciliation for short stay pateints

JCAHO has defined that Med Rec must take place when a patient has potential to recieve a medication- including Ambulatory Care areas- short term ED stay and Cath Lab.
Would be interested in beginning a dialogue on how to accomplish Med Rec in these areas yet not compromise patient flow.

Jill,
In reading the implementation expectation for this goal I interpreted that reconciliation should occur if the pt is being admitted to the organization. It does include Amublatory settings however I took that to mean you could start the process of collecting the medication information in the ED- please let me know where you saw the information that it must take place if the pt has a potential to receive a med. I am currently leading our implementation of med reconciliation and I had not included the ED dept, our plan focused on getting a detailed list once the pt was admitted to a nursing unit- please let me know
