Hello,
I just found this site, looked at the My Shared Care Plan. Would like to know how this is going after 2-3years? Maybe this isn't the place to ask and expect an answer. If so, please excuse.

Hi Lisa,
I just saw your post for the first time; sorry for the delay in my response!
Here's an update for you:
- We currently have 781 electronic Shared Care Plans
- We have shifted our focus to describe the tool as a "personal health record" rather than a "chronic disease management tool"
- Along with this change in focus, we have revised our expectations for clinician logins: it is no longer a requirement. Instead, we encourage patients to bring in printed copies to office visits, we are working on providing access to clinician-sourced data that patients can import into their Plan (like Quicken), and we are experimenting with ways to serve up patient-sourced data to clinicians in a way that fits in with their workflow (we have had some success with medication data in particular).
- We have expanded our implementation to include the towns of Eugene and Florence, OR

I believe the home health industry would be very interested in participating in this patient centric method of medical data sharing.
